I can only reproduce this with *telgate option.
I'm thinking this is not a *telgate issue but tossing it out just in case.
Overview:
I'm running two BBS' on the same server. One (SBBS) is on the standard telnet port and the other (Spitfire) is on 2323. Spitfire BBS is not fossil aware so I'm using Netserial as a virtual modem.
Question:
From SBBS i create a *telgate x-bit.org:2323 and i can connect just fine on the first try only. When i exit the Spitfire BBS and land back on the Synchronet BBS any additional tries to log back on won't work. My Spitfire BBS see's an attempt but the connection is not made. When I exit SBBS and log back on then it works gain but only the first attempt.
I can only reproduce this with *telgate option.

Another interesting twist, I can't reproduce this with netrunner, only syncterm.
What if you telgate to a *different* BBS (e.g. vert.synchro.net) - as a test, does that work multiple times?
