Who puts the name captions up on TV coverage of PMQs? I presume it is
done by the company doing the Parliamentary TV coverage and not BBC etc?
I have noticed two VERY obvious mistakes in identifying MPs this morning
and neither had any correction. I was not watching all the time and obviously do not know every MP so wonder how many more mistakes there
were.

One of the big reforms I'd like to see about the House Commons (the other is a*secret* ballot for all "division" votes *) would be an end to "the Honourable Member for Henley" terminology, and calling MPs by their names. Their names and their cabinet roles (if any) are*far* more important than their constituencies.
(*) And thus an end to the Whips office penalising MPs who vote against
their party on a particular issue because they are representing the views of their constituents.
