    I noticed several times in the recent (may still be current) run of "crimewatch", reference being made to "the number currently on your
    screen" or similar wording - when it wasn't. Often enough to be
    noticeable.

    I thought maybe it was just that prog., but it's common, and not just
    the BBC: I happened to be watching Sky News just now (a BBC report I'd
    seen one too many times drove me away - not the unpleasantness of the
    subject, just I'd seen it too many times), and heard something like
    "just scan the QR code currently on your screen" - needless to say,
    there wasn't one.

    Outgoing monitoring - with audio - is obviously rare these days.

    I have seen QR codes on Sky accompanying the weather with a
    "scan this for you local weather" caption, not noticed QR codes for news though.
