Malpositioned IOL

I have a sclerally-sutured IOL in the OD that is giving me a great deal of trouble. Perhaps because the eye has a vitrectomy, the inflammation has caused vision to become cloudy. This is controlled to a considerable extent by prednisolone eye drops, and (
because I am a steroid responder) acetazolamide tablets to control IOP.
The IOL will have to be removed. I have not yet had a pre-op appointment, but there are suggestions of leaving the eye aphakic at least for now. (If they can't do a better job than they did the first time, I'd be inclined to assent to this, although an
IOL is more convenient.) I would welcome advice here, particularly because I am a Kaiser patient and this limits my options to shop around for doctors.
