    I'm not sure what the answer is, but medical billing is the biggest fraud in history.

    For example, I just paid the bill for my wife's mammogram. The charge
    was $1100. Of that, we paid $35 co-pay, insurance paid $280 and $785 was "insurance adjustment." IOW, 71% of the charge is fluff so the insurance companies can tell the customers (employers) what a great deal we got for you. The real cost of the service is about $315. I wonder how much of
    the excess expense Americans pay for health care relative to other
    developed countries is "insurance adjustment" fluff. How many people get paid some kind of commission based on that fluff?

    Insurance adjustment an underrated value of high deductible plans. You get
    the adjustment whether you’ve reached your deductible or not.

    About 15 years ago I was in the hospital 48 hours and the bill was
    $42,000. Or $875/hr. Insurance paid way less than that.
