PADI website has the info; looks like there's three basic options:
a) Online, for a hardcopy replacement
b) Online, for an "e" (electronic online) replacement
c) Visit local PADI diveshop, where presumably they'll do one of the above for you

My preference would be (a) and when it arrives, take a digital photo of it; you can
then keep your own digital 'backup' copy this way...including making a printout of it.
What I've also done is to print out a copy of my C-Cards (yes, plural) and then taken my
dive log and taped that photocopy into it. I'll cover it with 2" wide clear packing tape to
effectively "laminate" it to make it a bit more waterproof.
When the dive log is full, I just print out another copy of the C-Cards & repeat.
