Are you talking about Merc's tyre strategy or the car's 'technical
error' that led to his disqualification?

    WHy not both?

That would imply 'strategies' rather than 'strategy'?

Not necessarily - a single failure of strategy can have multiple
    impacts.

Let's not get into nit-picking, though - I'm sure we've both had plenty
    of practice. ;-)

    The 'too late' tyre stop.

Hadn't heard about the skids things when I posted.

    LOL

Anyone know what the 'technical error' was?

    The 'too late' tyre stop.

I thought that Merc wanted to have enough tyre performance over the last
laps to catch Max who was on older tyres, and so extended Lewis' middle
stint. It was always going to be a guessing-game, and it almost worked.

Has anyone read the full report? Was it a marginal breach (possibly
explained by tyre wear) or more significant caused by running too
    marginal a setup?

    Here's what I got on ESPN.com: https://www.espn.com/f1/story/_/id/38722021/lewis-hamilton-charles-leclerc-disqualified-us-grand-prix
In a nutshell, the plank on the underside of HAM's and LEC's cars was worn too thin. The stewards acknowledged that the bumpy track and additional sprint race left the crews with less time to prep the cars for the Sunday GP, but both men were still DQ'd.
VER and NOR were also spot-checked and passed.

Wear on the boards, they were too low.
