If you want to do it, use a biopsy punch. Make sure you clean the area very well. If possible go to Walmart and get some betadyne cleanser(unless you're allergic to iodine containing products) and cleanse the area very well. Remember this area gets
sweaty and is warm, a perfect environment for bacteria. If you have any places in the genital area that look like they may already be infected(this includes yeasty infections) you should wait until these areas clear up. You do NOT want to get an
infection inside the scrotum! To make a viable hole you need to use at least 6mm(2g) punch. But before you use the punch it would be wise to go into a dark room with a good flashlight and use it to illuminate the scrotum. That way you can mark where the
piercing will be and avoid any of the larger vessels.
Because you have 2 flexible surfaces to hold together, you'll need some adhesive to hold them together, krazy glue. Hey that's why it was originally created! But now you have a problem, bleeding, and there will be some. For the adhesive to work the area
should be dry. You'll need to keep blood out of the area so use some type of circular apparatus to press down on the area restricting blood flow. You could even use a large circular key ring as long as it's been boiled for 10 minutes. The adhesive will
    be more effective that way.

Or you could try putting a piece of sterile gauze thru the hole.
