    I had a similiar problem and it was a poor connection from the power company's service drop to my service entrance wires. Power company replaced the wire connector and my problems were solved.

    It wasn't your fault, but you must have noticed the high torque when you loosened the lugs. Don't always go by the panel since the cables may not be what the manufacturer expected. I noticed a torque of about 150 instead of 42. You have to hold the lug
    with a strong cresent with one hand while using a breaker bar with the other to get the right amount with breaking the screws that hold the panel in the box.
