My desktop Chrome browser icon is as it should be; so is the icon on
Quick Launch toolbar, but when Chrome is maximized or minimized, the
icon on the bottom left corner is a white filled rectangle.
Why, and how do I fix it?

Be a while since I last used Chrome, but do not remember there was a
Chrome icon at the lower left corner of its window. In a Google Image
search on Chrome, I still don't see a Chrome icon in the bottom left
corner of its window.
The bottom left corner is where the status bar appears (when it has some status to display). The status bar should appear showing the URL string
of a hyperlink when you hover the mouse pointer over a hyperlink in a
web doc. This is to prevent phishing attacks by showing a hyperlink pretending to point to a well-known site, but the status bar showing to
where the hyperlink really points. The status bar will show loading resources for a web doc.
Are you displaying a blank web page (about:blank), or some web site when
the icon is missing? about:blank should show a completely blank doc
window. If you're loading some other page, maybe it is that web page
that removed or unlinked the icon, or a resource cannot be reached
(e.g., when using an adblocker).
Take a screenshot of the Chrome window, upload to online storage (e.g., imgur), and give a URL, so we can see what you see.

I may do a screenshot later, but yes, the icon on the Statusbar is the
one that appears as a white rectangle rather that the usual coloured
disc that appears on the Desktop and Quicklaunch toolbar where the
coloured disc does appear correctly. BTW when I click on the 'white rectangle' the correct webpage appears. It's just that icon thats wrong.

I thought the icon, if one appears, was the site's favicon.ico. If the
site doesn't have one, there isn't one to show.

That shows the status bar appearing when the mouse cursor hovers over a hyperlink in the web doc. That doesn't have an icon: no Chrome icon,
and no favicon for the web site that hasn't yet been visited.
When resources are slow enough when getting retrieved, like scripts,
